{"id":3447,"date":"2022-10-05T14:47:24","date_gmt":"2022-10-05T14:47:24","guid":{"rendered":"https:\/\/drdimolenis.gr\/osteoporos\/"},"modified":"2022-10-05T14:47:24","modified_gmt":"2022-10-05T14:47:24","slug":"osteoporos","status":"publish","type":"post","link":"https:\/\/drdimolenis.gr\/sv\/osteoporos\/","title":{"rendered":"Osteoporos"},"content":{"rendered":"<p>Sjukdomsf\u00f6rloppet kan beskrivas som l\u00f6mskt eftersom det inte visar symptom f\u00f6rr\u00e4n en fraktur uppst\u00e5r. De allvarligaste osteoporotiska frakturerna \u00e4r de i l\u00e5rbenets huvud och hals. Under de f\u00f6rsta tre m\u00e5naderna efter frakturen uppst\u00e5r komplikationer i 40 % av fallen och d\u00f6dligheten fr\u00e5n dem \u00e4r 25 %. F\u00f6rutom sjuklighet \u00e4r kostnaden (social, psykologisk, ekonomisk) f\u00f6r frakturen p\u00e5 grund av eventuell funktionsneds\u00e4ttning skr\u00e4mmande.   <\/p>\n<p>Av ovanst\u00e5ende \u00e4r det l\u00e4tt att se att osteoporos tillh\u00f6r den grupp sjukdomar d\u00e4r f\u00f6rebyggande \u00e5tg\u00e4rder \u00e4r av st\u00f6rsta vikt. Behandling av etablerad osteoporos kan minska risken f\u00f6r frakturer, men det \u00e4r osannolikt att den helt \u00e5terst\u00e4ller benkvalitet och styrka. <\/p>\n<p>Det finns tv\u00e5 viktiga begrepp f\u00f6r att f\u00f6rst\u00e5 f\u00f6rloppet av osteoporos: den h\u00f6gsta densiteten som benet f\u00f6rv\u00e4rvar under en persons liv (Maximal bent\u00e4thet) och den hastighet med vilken den f\u00f6rloras med \u00e5ldern. \u00c5ldern d\u00e5 maximal bent\u00e4thet uppn\u00e5s (dvs. n\u00e4r benet har st\u00f6rst styrka) \u00e4r inte k\u00e4nd med s\u00e4kerhet, men placeras vanligtvis i det tredje levnadsdecenniet f\u00f6r de flesta. Ju h\u00f6gre maximal bent\u00e4thet \u00e4r vid denna \u00e5lder, d.v.s. ju t\u00e4tare och starkare benet \u00e4r d\u00e5, desto starkare kommer det att f\u00f6rbli, proportionellt alltid, vid 60 eller 70 \u00e5rs \u00e5lder. F\u00f6rlusthastigheten \u00f6kar fr\u00e4mst hos kvinnor efter klimakteriet, n\u00e4r \u00f6strogenets skyddande effekt upph\u00f6r.   <\/p>\n<p>Studier p\u00e5 monozygota tvillingar har visat att h\u00f6jden av maximal bent\u00e4thet best\u00e4ms genetiskt till en procentandel av 60-70%. De \u00e5terst\u00e5ende 30-40% best\u00e4ms av milj\u00f6faktorer, s\u00e5som kost, motion, vanor, sjukdomar och medicinering. Professor Charles Dents uttalande att &#8221;senil osteoporos \u00e4r en pediatrisk sjukdom&#8221; \u00e5terspeglar hur viktigt det \u00e4r att uppn\u00e5 h\u00f6gsta m\u00f6jliga maximala bent\u00e4thet i ung \u00e5lder f\u00f6r b\u00e4sta m\u00f6jliga bentillst\u00e5nd senare i livet.  <\/p>\n<p>Personer med \u00f6kad risk att utveckla osteoporos \u00e4r fr\u00e4mst kvinnor med klimakteriet (naturligt eller efter operation) f\u00f6re 45 \u00e5rs \u00e5lder samt personer med sk\u00f6ldk\u00f6rtelsjukdom, personer som genomg\u00e5r behandling med glukokortikoider eller heparin och kvinnor med l\u00e5ngvarig menstruationsbrist, hypotalamisk etiologi. Dessa personer b\u00f6r genomg\u00e5 en bed\u00f6mning av risken f\u00f6r att utveckla osteoporos. Det b\u00e4sta testet vi har \u00e4r m\u00e4tning av bent\u00e4thet med DEXA. Detta \u00e4r ett enkelt och sm\u00e4rtfritt test med l\u00e5g kostnad och minimal radiologisk belastning f\u00f6r personen. Diagnosen osteoporos (eller dess f\u00f6rstadiumtillst\u00e5nd som kallas osteopeni) st\u00e4lls sedan genom statistisk j\u00e4mf\u00f6relse av patientens m\u00e4tningar med m\u00e4tningar fr\u00e5n den allm\u00e4nna befolkningen, av samma k\u00f6n, ung (t-po\u00e4ng) eller samma \u00e5lder (z-po\u00e4ng).    <\/p>\n<p>F\u00f6rebyggande av osteoporos fokuserar p\u00e5 tre huvudaxlar:<\/p>\n<ul>\n<li>N\u00e4ringsintag. Tillr\u00e4ckligt intag av kalorier, kalcium fr\u00e5n mejeriprodukter och D-vitamin kr\u00e4vs. Dessa tv\u00e5 element (kalcium och D-vitamin) \u00e4r viktiga f\u00f6r att bibeh\u00e5lla korrekt bent\u00e4thet och struktur. Minskad alkoholkonsumtion rekommenderas ocks\u00e5, \u00e4ven om det inte har klarlagts om minskad koffein- och saltkonsumtion hj\u00e4lper eller inte.  <\/li>\n<li>Motion. Det rekommenderas att tr\u00e4na minst 3 g\u00e5nger i veckan i 30 minuter. Promenader \u00e4r ett utm\u00e4rkt tr\u00e4ningsalternativ. I vilket fall som helst b\u00f6r typen av tr\u00e4ning inte vara s\u00e5dan att den predisponerar f\u00f6r fall och skador. F\u00f6rdelarna med tr\u00e4ning varar s\u00e5 l\u00e4nge personen tr\u00e4nar. \u00d6verdriven tr\u00e4ning kan \u00e5 andra sidan leda till att hypotalamus funktion undertrycks och d\u00e4rmed \u00f6ka risken f\u00f6r att utveckla bensk\u00f6rhet.     <\/li>\n<li>Att sluta r\u00f6ka.
